The Don Mills Family Health Team is an innovative provider of primary health care programs and services for the residents of Don Mills and surrounding neighbourhoods in Toronto. We bring together different health care professionals to co-ordinate and deliver the highest possible quality of care to you – our patients – and to promote healthy living.
Our Vision is to become a leading community-based primary health care organization with a special focus on seniors and immigrants. It will provide innovative, multi-disciplinary patient-centred primary health care, emphasizing continuous quality improvement, health promotion and disease prevention, and focused on the social determinants of health and well-being.
Our Mission is to enhance the health and well-being of individuals and improve population health through accessible, innovative, responsive and close to home services delivered in partnership with our community.
Our Goal is to advance the development of an overall effective primary health care system, with special emphasis on the target groups of seniors and immigrants who comprise a large portion of the local population.
